Root cause analysis (RCA) implies it's *always* fair to place 100% blame on one specific act. In software, as in life, that's naively simple. There are normally multiple independent factors. RCA creates unfair findings, bad decisions, and anger/distrust. Operations get riskier.

This stunning stag was shot dead by Police in Merseyside after cornering it in an industrial estate. They were advised by RSPCA to leave it and it would find its way home but they claimed couldn’t get any organisation to come and help. A vet was called but instead they shot it 😡
