I'd like to replace the word "requirement" with the word "conversation". Instead of reaction to a bug being "we missed that requirement", what if it was "we missed that conversation". Because we did, we missed the conversation. And conversations are on *us*.

A hacker is someone who gains unauthorized access to information or content. This individual did not have permission to do what they did. They had no authorization to convert and decode the code.
